Housing Based Case Management

With case management clients receive personalized help while homeless and in housing.

What is Housing Based Case Management?

Housing Based Case Management is for individuals who struggle with complex barriers that prevent them from getting housing. These barriers could include:

  • Poverty
  • Physical or mental health issues
  • Drug Use
  • Immigration status
  • Issues with past housing, such as eviction
  • Criminal record

Housing-based case management aims to house those high needs persons in stable, permanent housing situations. This is an intensive service that brings together multiple non-profit and government organizations to untangle the complex issues that are keeping a person from permanent housing.

Case management addresses the most urgent issues to get a client ready for housing and once in an appropriate permanent housing situation, case management can continue to support the client in addressing chronic, complex issues and keep them housed.

How it works

Case managers and clients create a personalized action plan to connect them to permanent housing. Depending on a person’s needs and goals, an action plan may include access to behavioral health treatment, reengagement with family, wraparound case management or other assistance, all with the goal of placing that person as quickly as possible in permanent housing appropriate for their needs. A key aspect of housing-based case management is regular on-going case conferencing meetings, where our partners meet to coordinate care and address immediate client barriers to housing.
