Get a medical marijuana ID card

Need to know

  • Applications are processed by appointment only.
  • The state card fee is $100. Medi-Cal patients pay $50. CMSP patients pay $0.
  • Office hours are Monday to Friday, 9 a.m. to 4 p.m., closed 12 p.m. to 1 p.m.

Before you start

  • Download and complete application or get an application packet from the Vital Statistics Office.
  • Have a valid photo ID.
  • Gather proof of Marin County residency (for example, a utility bill, rent or mortgage receipt, or CA vehicle registration in your name).
  • If you have Medi-Cal, bring proof for the reduced fee.

What to do

  1. Call the Vital Statistics Office at 415-473-3288 to request an application packet and schedule an appointment.
  2. Complete the application, including the physician section.
  3. Go to your appointment at 10 N. San Pedro Road, San Rafael, CA 94903.
  4. Bring your photo ID, proof of residency, and proof of Medi-Cal status (if applicable).
  5. Pay the fee, and have your photo taken.

After your appointment, the state issues the card. You can pick up your card at the Vital Statistics Office after it arrives.

  • Renewals require a new appointment and fee. No expiration notice is sent.
  • Report lost or stolen cards right away, and reapply for a replacement.
