Before you start
- Proof of identification (driver's license, immigration card, state ID card, passport, or photo ID from another country)
- Proof of HIV diagnosis (diagnosis letter from doctor's office on MD stationery, lab test result showing viral load, or positive test result)
- Proof of Marin County residency (utility bill, lease/mortgage statement, support affidavit, or letter from a shelter)
- Proof of income (state/federal tax return indicating adjusted gross income, W-2 or 1099 form, current pay stub, bank statement indicating direct deposited income, disability award letter, self-employment affidavit, or support affidavit)
