Need to know
- The Medical Therapy Program (MTP) is part of California Children's Services (CCS). It helps children who have physical disabilities.
- The program may provide:
- Physical therapy (PT)
- Occupational therapy (OT)
- Medical Therapy Conference (MTC) services
- A Medical Therapy Conference (MTC) is a team meeting. The team includes:
- The child and their family or caregiver
- A physical therapist
- An occupational therapist
- A doctor
- The team works together to plan the child’s care.
Before you start
- Have these ready:
- Your child’s medical records
- Any past therapy reports
- Your child’s school name
- IEP (Individualized Education Plan), if your child has one
What to do
- Confirm your child qualifies for CCS
- Get a referral to CCS
- Fill out a CCS application
- Send medical records
- Go to your first appointment
- Attend a therapy evaluation
- Follow your child’s care plan
- Confirm your child qualifies. To qualify, your child must be eligible for California Children’s Services.
- Get a referral to California Children’s Services from a parent, teacher, doctor, or case manager.
- Submit a signed California Children’s Services application by phone, mail, or fax.
- Send medical records from your child’s doctor or hospital for eligibility review.
- If eligible, attend the first program appointment to complete forms.
- Attend the therapy evaluation and follow the plan set with the therapist(s).
