- Enrollment is during pregnancy or before the baby is 3 months old.
- Services can continue until the child’s 5th birthday.
- Home visits are offered 1 time per week for the first 6 months after birth, then adjust based on needs.
Before you start
- Have parent or caregiver contact information ready.
- Be ready to share basic information about the family’s needs.

Submit a referral
- Open the Healthy Families Marin Referral Form.
- Complete the form and submit it.
- Let the family know you are making a referral.
Self-refer
- Parents and caregivers can submit the referral form for themselves.
