Need to know
Health care providers are required under California law (Title 17) to report:
- Suspected or confirmed active TB cases (report immediately; do not wait for lab confirmation)
- Positive TB tests in children under age 5
- TB test conversions within the past two years
Hospitals must complete a TB Discharge Treatment Plan before patient discharge.
Before you start
- Have patient demographics and contact information
- Gather clinical details and risk factors, if available
- Do not wait for laboratory confirmation to report
What to do
- Submit a report
- Complete the Confidential Morbidity Report (CMR) form (CDPH-110b) for all suspected and active TB cases among Marin County residents.
- Fax reports to 415-473-6002.
- Report urgently if needed
- During business hours (Mon to Fri, 9 a.m. to 5 p.m.) call 415-473-4163 or email Marin Communicable Disease
- After hours, call the Health Officer on Duty at 415-499-7238.
- Ensure all suspected and confirmed TB cases are reported promptly
- Include complete and accurate clinical information when available
