Choose a service and pay

1. Use the tick payment portal for tick identification, or the well water testing portal for water tests.
2. If you pay online, write the receipt number on your requisition form.

Submit your sample

1. For tick testing, mail your tick and completed requisition to the laboratory address listed on the source page.
2. For private well water testing, pick up prepared bottles in San Rafael, collect your sample, keep it cold, and drop it off during posted hours.
