Before you start
- Write down the suspected disease, when it happened, and where it was reported.
- Gather name of patient and details of lab tests.
- Include close contacts, if you have them.
What to do
- Review the Communicable Disease Prevention and Control page for the correct reporting process for the issue.
- If you are a health care provider and the issue is urgent outside of business hours, follow the after-hours instructions.
- If you are a community member or provider and the issue is not urgent, use the email or phone option listed for the program.
